We complete school, sports, and work physicals and the forms that go with them for patients age 12 and older. For employers and workers we also do pre-employment exams and return-to-work evaluations, including the return-to-work note.

We see patients age 12 and older, including for school and sports physicals.
Do you do pre-employment and return-to-work exams?
Yes. We do pre-employment exams and return-to-work evaluations, and we write the return-to-work note when you are ready. Bring any forms from your employer.
Do you complete school and sports forms?
Yes. Bring the form with you so we can complete it at the visit when we can.
